A Lovin Touch - Pet Care

Every pet I bring to A Lov-in Touch(ALT) has received all shots required by the city/county where I reside plus bordetella drops and I will bring proof of such.
I will not bring to ALT any pet with an illness or condition that might affect other pets.
No pet brought by me to ALT has ever been known to bite or attack humans.
I will provide my pet's food in a CONTAINER while at ALT. I agree that ALT is not responsible for any dog related items, ie toys, bones, leashes, beds, etc. as other dogs ay play with them.
I will tour the facility and understand that the dogs at ALT play in the yard together for exercise and enjoyment. Please make sure you tell us about any of your pet's idiosyncrasies such as: jumps over or digs under fences, opens gates, may be aggressive, howls at night, chews on beds, afraid of thunder, loves kisses, any medications, etc. I understand that if my dog has the ability to jump/climb a fence of this height, I am liable if my dog escapes.
I will be responsible for any harm my pet causes to other pets, people or property.
All dog play is carefully monitored by ALT staff to avoid injury, however scratches, punctures, torn ligaments, etc. may occur despite the best supervision. I understand and agree that ALT, its employees, staff, or volunteers will not be held liable for any illness, injury, death, and/or escape of your pet.
I authorize ALT to obtain veterinary treatment for my pet if necessary. I will be responsible for all veterinary bills and will not hold ALT responsible for any results of such treatments.
I will not bring a female dog in heat or an intact male to ALT.
My dog understands basic commands. My dog has a collar (not a choke chain) and a tag with his name on it. My dog has good manners, the two most important being: HE DOES NOT JUMP ON PEOPLE AND DOES NOT BARK EXCESSIVELY. Both these habits can be reason for refusal for boarding or daycare at a ALT.
I understand that ALT uses a mixture of 1/4 vinegar & water in a spray bottle to train dogs and a spray bark collar to prevent barking.
I will pay my bill upon picking up my dog on Fridays for day care or on the last day the service is performed with cash, check or credit card. I will receive a 10% family discount for more than one pet and a 5% discount for cash payments with invoices over $100. Should I choose to have my pet(s) valeted, I agree to pay in advance.
Low Season Rates area as follows: overnight stay/dog $28, overnight stay/cat $10, Doggy daycare $20.
High Season Rates are as follows:overnight stay/dog $35, overnight stay/cat $15. Holidays include President's day weekend, Thanksgiving, Christmas, Spring Break, Memorial day, the Fourth of July, and Labor Day.
I am aware that the day I "check" my pet for boarding, I will receive an overnight charge whether drop off is at 7:30 a.m. or 6:00 p.m. and that "check out" is before 9:30 a.m. to avoid an additional day care charge.
I will abide by the posted hours of ALT: M - F 7:30 a.m. - 9:30 a.m.& 4:30 p.m. - 6:30 p.m. SAT 9:00 a.m. - NOON; Closed Sunday for family and worship time.
Please provide arrangements for pets should anything happen to you and arrangements should your pet pass away while boarding (required by State).
I understand that the cost for the Trial Day is $20 and you will receive a report card and a tour that afternoon.
